Dental Lab Return Policy

At  Qualitydenture.com (DBA Levy 4581 LLC), we are committed to providing exceptional dental prosthetics and appliances that meet the highest standards of precision, quality, and patient satisfaction. We understand that from time to time, a remake or return may be necessary. This policy outlines our return and remake procedures to ensure clarity and consistency.


1. Eligibility for Return or Remake

Products are eligible for return or remake if they meet one or more of the following criteria:

  • The item has a verifiable manufacturing defect.
  • The appliance or prosthetic does not match the prescription specifications (e.g., shade, material, design).
  • The item was damaged in shipping and reported within 3 business days of delivery. 


2. Non-Returnable- Items

We cannot accept returns or provide free remakes in the following situations:

  • Inaccurate or incomplete prescriptions or impressions.
  • Clinical or impression errors not caused by lab fabrication.
  • Appliances that have been altered, adjusted, or repaired after delivery.
  • Products returned more than 30 days from the original delivery date without prior authorization.


3. Return and Remake Procedure

To initiate a return or remake request:

  1. Contact the Dental Lab Coordinator within 7 business days of receiving the item.
  2. Provide the case number, patient name, and a detailed description of the issue.
  3. Include photos or documentation, if applicable, to assist with evaluation.
  4. Upon approval, a Return Authorization (RA) number will be issued.

All items returned without an RA number will not be accepted or credited. 4. Shipping Instructions

  • Clearly write the RA number on the package.
  • Use original or secure packaging to prevent further damage.

  5. Turnaround Time

  • Remakes due to lab error: Prioritized and typically completed within 5–7 business days.
  • Remakes due to clinical error or impression issues: Standard turnaround applies, and may incur remake fees. 


 6. Fees and Credits

  • No charge for remakes due to lab error.
  • Remake fees may apply for clinical or prescription-related errors.
  • Account credit will be applied once the returned item has been received and inspected.
